- Arranges transportation to off-site healthcare appointments
Individuals who are enrolled in our Medicare Advantage Plans are provided access to a Care Ally who:
- Monitors care and treatment plan progress
- Visits residences regularly to answer any questions and provide support
- Manages care transitions
- Identifies and addresses any care gaps
- Schedules specialty care, lab work, and imaging appointments
- Fully reviews health plan benefits to help individuals gain access to additional support services
