Revolutionizing Senior Care: The Power of Value-Based Care Technologies

In the ever-changing senior housing landscape, skilled nursing facilities and assisted living communities are under increasing pressure to deliver exceptional healthcare for their residents while optimizing costs and enhancing operational performance.

Fortunately, new HIPAA-compliant technologies are meeting these challenges head-on, with value-based care organizations (like Curana Health) making them accessible through innovative partnership models.

Why Value-Based Care Needs Modern Technology

Recent technological advances in senior living have produced powerful solutions that enhance resident care, protect work-life balance, and improve operational efficiencies, including:

  • Real-time intelligence applications that analyze patient data to flag potential health risks, provide instant notifications regarding transitions between care facilities, and share medical history updates for timely care planning
  • Secure clinical communication networks which connect entire care teams in real-time, creating seamless information flow that enhances decision-making and leads to better resident outcomes
  • Remote patient monitoring systems that track residents’ vitals and movement patterns via radio wavelengths, alerting clinicians to concerning changes

Why Partnering Matters: Value-Based Care Organizations Deliver Ability, Not Burden

Dedicated to enhancing patient outcomes and prioritizing quality care, many value-based clinical care organizations have already implemented these advanced technologies into their workflows.

Through partnerships with organizations that have already invested in these sophisticated tools, senior housing communities can gain immediate access to transformative technology without the burden of implementation costs and maintenance. They also benefit from expert resources who have prior experience with all stages of the implementation process, providing built-in resources for support at every stage.

This is just one of the many benefits of value-based care, which has been proven to improve resident outcomes and satisfaction, reduce staff burnout, and enhance operator revenue opportunities.

Introducing LINC: Curana’s Secure Transition & Communication Tool

In addition to advanced remote patient monitoring technologies, an integrated electronic health record, and software for enhanced chronic care management, Curana Health is now offering our partners a suite of technologies dedicated to streamlining communications between hospital systems, skilled nursing facilities, assisted living communities, and patients.

Known as Live Information Network Connection (LINC), this solution is a first-of-its-kind, HIPAA-compliant texting platform that includes care transition and discharge alerts coordinated across admitting organizations, residents, care teams, and providers.

By instantly alerting care teams when residents move between settings and enabling secure messaging between providers, this streamlined platform eliminates communication gaps and ensures everyone has the information they need to make timely care decisions.

As LINC rolls out to partner organizations nationwide, it will also become an integral part of Curana Health’s standard partnership package (providing operators with extra layers of staff and resident support).

For more information on LINC and Curana’s other offerings, including on-site primary care services and value-based care solutions (Accountable Care Organizations and Medicare Advantage Special Needs Plans), be sure to email [email protected].
