Annette Receives Compassionate, On-Site Healthcare Through Curana Health

A retired teacher and professor, Annette Crotchfelt, 80, has lived a very full life. In 2018, she and her husband, Fred, decided it was time to take the next step in their adventure together: moving into a senior living community.

“In our search for the right fit, there were a few things that we saw as important,” Annette said. “One was that we needed to have access to on-site healthcare services while still seeing our longstanding family doctors.”

Presbyterian Senior Living’s Ware Presbyterian Village offered almost everything the Crotchfelts were looking for and then some (including many exercise classes to help them stay in shape). Although the community didn’t offer a full range of healthcare services at the time, Annette and Fred were hopeful that would eventually change (and felt better knowing there was an on-site wellness nurse who could provide support if needed).


They quickly moved into the community, starting the next stage of their lives and looking forward to the new experiences that were awaiting them.

Turning to Curana Health for convenient care

In 2023, the Crotchfelts’ wishes were answered when Curana Health brought a full range of on-site healthcare services to Ware Presbyterian Village. Offering primary, urgent, and secondary care services (which allow residents to continue seeing their current primary care provider for routine needs), residents can choose the level of care they wish to receive through Curana Health’s providers.

While Annette had continued seeing her off-site primary care doctor, she decided to give Curana Health a try when she had difficulty scheduling an appointment with her regular care team.

“At the time, my primary care doctor wanted me to have my blood pressure taken two to three times a week, but all of their appointments were booked, and I couldn’t get in.” Annette said. “I didn’t know where to go, but then I remembered we have access to care right here in our community!”

Annette began seeing Curana Health’s providers multiple times a week for her blood pressure checks.

“After those first interactions, I began turning to Curana for health concerns more often,” Annette said.

In one instance, Annette made an appointment with Curana Health for a suspected infection. “They diagnosed me, prescribed an antibiotic, and sent a detailed report to my primary care doctor electronically,” she said. “The whole experience was so convenient, and even better, I didn’t have to lift a finger to keep my other doctor in the loop.”

Rather than making the trip to an off-site doctor’s office when she needs immediate medical care, Annette often contacts Curana Health first.

“While they’re not my only care team, they provide excellent urgent care services and health support when I don’t feel like traveling off-campus,” Annette said. “It’s so reassuring knowing that the Curana team is available 24/7 if I’m not feeling good and need to speak to someone. And, if the care I need is beyond what they can provide—like when I needed my hip replacement—they can refer me to a specialist and still stay in touch to make sure I am doing okay.”

Dr. Chao’s expertise spans a range of healthcare models, from fee-for-service to value-based care across Medicare, Medicaid, and commercial plans. His broad experience has honed his ability to lead both clinical and operational teams through the complexities of healthcare delivery models, and his professional insights are shaped not only by his clinical expertise, but also by personal experience (having witnessed firsthand the challenges his elderly parents faced in navigating the American healthcare system).
