In 2023, the community strengthened its commitment to that mission even further, partnering with Curana Health to bring value-based, on-site primary care services to residents and patients across all levels of care.
More than a general wellness initiative, this comprehensive, on-site care collaboration has fundamentally changed how healthcare is delivered and experienced at Ware Presbyterian Village, helping residents and patients better manage their chronic medical conditions, prevent emergency room visits and hospitalizations, and stay in their current level of living longer—all without ever having to leave the community they call home.
A New Standard of Care
Before Curana Health came to Ware Presbyterian Village, independent living and personal care residents often had to travel off-site for primary and urgent care needs: a challenge that extended far beyond simple inconvenience.
“A lot of our residents don’t drive anymore, so it was a significant challenge for them to arrange transportation to their appointments,” explained Katie Nemeth, Ware Presbyterian Village’s Independent Living Resident Services Director. “Even when they could arrange transportation, they often couldn’t get appointments for weeks. That’s a problem for anyone, but it is especially concerning for older adults with complex health needs.”
That’s where the partnership with Curana Health filled a critical gap: bringing a wide range of primary and urgent care services—including preventive care, chronic disease management, sick/injury visits, comprehensive care coordination, and 24/7 access to care teams—to residents within their own community.

“When Curana first arrived, some residents were hesitant about using someone new for urgent or primary care. But over time, word spread about how kind, helpful, and available their providers were, and more and more people started to give them a chance,” said Ware Presbyterian Village’s Executive Director Michelle Loucks. “We had a resident with a medical condition who couldn’t get in with her off-site primary care doctor for weeks. When she called Curana, they saw her right away and helped her get the prescription she needed very quickly. That’s just one of the many success stories we’ve heard.”
And it’s not just independent living and personal care residents who are happy with their care through Curana Health. The organization also provides comprehensive medical directorship services and rounding providers within Ware Presbyterian Village’s skilled nursing facility, overseeing and providing care for the community’s highest-acuity residents.
“Our Curana Health medical director is deeply committed to antibiotic and overarching medication stewardship,” Loucks said. “Her thorough documentation and commitment to evidence-based prescribing helps us stay compliant with the latest regulations and keeps our most vulnerable patients safe from antibiotic resistance.”
Benefits Beyond the Bedside
The benefits of having Curana Health on-site extend beyond patient care to include meaningful support for Ware Presbyterian Village’s care teams.
Their providers participate in care transition meetings and bi-weekly leadership calls with Ware Presbyterian Village’s directors from every level of care.
“Those regular check-in calls are so important,” Loucks said. “Even if it’s just a few minutes, it ensures everything is going smoothly. It’s a true partnership between the providers and operations team.”
Additionally, the Curana Health presence on-campus has played a significant role in selling the community to potential residents and their family members.

“Whenever I meet with potential and new residents, I tell them about the comprehensive healthcare services that are available to all our residents,” Nemeth said. “They love hearing that there’s a medical provider right here, even if they’re very healthy and don’t think they’ll need to use the service often. It’s very comforting and provides peace of mind.”

As more residents and patients begin to adopt Curana Health for care, both teams are proud to reflect on the partnership’s early achievements.
