On the origin of the program

CMS set up the Medicare Shared Savings Program so medical groups get paid for keeping patients healthier and driving down total cost of care. Since most of that care happens where patients live, ACOs commonly share a portion of their savings with the senior living communities that help coordinate it.

Read an ACO's CCSA like any clinical services agreement: what services it covers, how you get paid, how long it runs. We will send the whole thing over and go through it with your team.

On the math

No savings, no payment. Most ACOs do not generate savings every year, so a bigger portion of zero is still zero. Your check comes down to two things: how much the ACO saved, and then what share of it you get.

PCMA Collective, Curana's MSSP ACO, sits in the top 1% nationally and generated upwards of $110M in Medicare savings last year.

So ask about track record before you ask about percentage, because performance is what actually generates a paycheck.

Derek Chao, MD

President and Chief Medical Officer
Dr. Chao brings more than two decades of experience in managed care, clinical leadership, and healthcare transformation to Curana Health. Starting his medical career as a hospitalist and nephrologist in Southern California, Dr. Chao has held various leadership positions (including medical director and chief medical officer) within numerous healthcare organizations (including HealthCare Partners). Before coming to Curana, he served as chief executive officer of Optum Health’s western region, where he oversaw operations and care delivery across nine states. Dr. Chao’s expertise spans a range of healthcare models, from fee-for-service to value-based care across Medicare, Medicaid, and commercial plans. His broad experience has honed his ability to lead both clinical and operational teams through the complexities of healthcare delivery models, and his professional insights are shaped not only by his clinical expertise, but also by personal experience (having witnessed firsthand the challenges his elderly parents faced in navigating the American healthcare system).
