Care Management with Curana Health

Curana Health’s Care Management programs provide extra support for residents and their families through a proactive, coordinated approach to care. These programs focus on prevention, early identification of health needs, and continuity of care, supported by personalized care planning, ongoing monitoring, and clear communication. Delivered by an interdisciplinary team—including providers and on-site or virtual support staff—Care Management helps reduce avoidable health issues and unnecessary hospital visits.

Patients of Curana Health have access to these Care Management Programs

Curana Health's APCM program is a monthly service covered by Medicare that gives you extra support between doctor visits. Our team of trained nurses helps keep your providers coordinated, catches changes in your health early, builds a personalized care plan around your goals, and supports you through hospital stays and the transition back home.

Behavioral Health Integration (BHI) adds emotional and mental health support right into your regular care. A dedicated Curana Health Care Coordinator checks in with you regularly, works with your doctors and nurses, and can keep your family informed — so your whole health is looked after, not just the physical.

Chronic Care Management (CCM) is a Medicare-supported service, offered through Curana Health, and designed for residents living with multiple chronic conditions. CCM provides preventive and proactive support to help patients manage chronic conditions between doctor visits.

Backed by the Centers for Medicare & Medicaid Services, the GUIDE Model (Guiding an Improved Dementia Experience) provides expert care, compassionate support, and practical resources to help you and your loved ones live more comfortably and confidently.

Leaving a hospital or facility comes with a lot to keep track of—medications, follow-up visits, and new routines. Curana's Transitional Care Management (TCM) program gives you extra support during this time, so you don't have to manage it alone. You'll have a dedicated care team checking in with you, answering questions, helping you understand what comes next, and working closely with your primary care provider, so your transition feels smoother, safer, and more comfortable while you focus on your recovery.

Derek Chao, MD

President and Chief Medical Officer
Dr. Chao brings more than two decades of experience in managed care, clinical leadership, and healthcare transformation to Curana Health. Starting his medical career as a hospitalist and nephrologist in Southern California, Dr. Chao has held various leadership positions (including medical director and chief medical officer) within numerous healthcare organizations (including HealthCare Partners). Before coming to Curana, he served as chief executive officer of Optum Health’s western region, where he oversaw operations and care delivery across nine states. Dr. Chao’s expertise spans a range of healthcare models, from fee-for-service to value-based care across Medicare, Medicaid, and commercial plans. His broad experience has honed his ability to lead both clinical and operational teams through the complexities of healthcare delivery models, and his professional insights are shaped not only by his clinical expertise, but also by personal experience (having witnessed firsthand the challenges his elderly parents faced in navigating the American healthcare system). At Curana Health, Dr. Chao leads efforts to empower the organization’s healthcare providers, optimize care delivery, and ensure clinicians have the tools and support needed to achieve exceptional outcomes for patients and operator partners alike.
